The nurse assesses the skin by visual inspection for any changes in appearance such as tumor or cysts, redness, scaling, and oozing. Additionally, the nurse may use palpation techniques to assess for any tenderness or swelling and use specific instruments to measure color changes, moisture or temperature of the area.

Nursing Diagnosis

The nurse's diagnosis can be based on the results of the assessment along with observations of the patient's behavior and responses. Common nursing diagnoses include risk for impaired skin integrity, impaired skin integrity, impaired wound healing and activity intolerance.


The patient should be able to demonstrate an increased knowledge regarding skin care and prevention of skin breakdown. The patient should also be able to maintain intact skin integrity and demonstrate improved wound healing. Additionally, the patient should demonstrate an increase in tolerance to daily activities.


The interventions for skin integrity that the nurse will likely perform are therapeutic treatments, grooming, nutrition, medication management, supportive treatments, and education. Therapeutic treatments may include wound care, cleansing, and application of topical medications. Grooming includes clipping nails, brushing teeth, and checking for areas at risk for pressure ulcers. Nutrition can help promote skin health by providing the body with the essential vitamins, minerals and proteins it needs for healing. In addition, medication management, as prescribed, can be used to treat wounds or infections.


The rationales for all the skin care interventions are to prevent or minimize the risk of skin breakdown, infection, and scarring. Therapeutic treatments are designed to clean, soothe, and protect the wound and skin. Proper grooming reduces the risk of infection and irritation. Nutrition strengthens the skin and helps wounds to heal faster. Medication management can be used to reduce bacterial growth or penetration of skin.


The evaluation should be focused on the effectiveness of the interventions and the patient's ability to maintain intact skin integrity. The nurse should monitor the patient's skin on a regular basis and document any changes in the skin. Additionally, the nurse should assess the patient's knowledge level and satisfaction with the care that was provided.
